This General Physical Exam was developed by Dr. Don Nelson at the Cedar Rapids Medical Education Foundation in Cedar Rapids Iowa. This single page Physicial exam includes vitals and 14 different systems for review. Dr. Nelson uses a combination of visibility, radio buttons and check boxes to document each system. Rather than using the complains of/denies model, Dr. Nelson has developed text that more specifically describes the exam findings. The most interesting feature of this form is a set of radio buttons that allow the user to choose the formatting of abnormals and comments using radio buttons. NOTE: Because some of the files included with this form are larger than 32K, it cannot be imported into Logician Watcom Single User Training. Please import this form into network training for testing.
