NCVHS - National Committee on Vital Health Statistics, the Uniform Ambulatory Care Data Set. Standards for data collection.

Health Level 7 - Standards for the electronic interchange of clinical, financial, and administrative information among independent healthcare oriented computer systems, such as hospital information, clinical laboratory, enterprise, and pharmacy systems.
